Apply for, replace, transfer or add a class to a high risk work licence in Tasmania.

- You must have a licence to operate certain types of high risk equipment. There are a number of classes of high risk licence.
- Before you apply, or add a new class to an existing licence, you must have completed a recognised course of training with a registered training organisation and been formally assessed as competent.
- You must apply in person at a service centre, with a completed application form (PDF, 344KB), your assessment, interim summary assessment and proof of identity. Your licence is valid for 5 years.

About applying for, replacing, transfering or adding a class to a high risk work licence
You must have a licence to work with high risk equipment. There are a number of classes of equipment and plant that are covered by a high risk work licence.
The only time you do not need to apply for a licence is when you are:
- formally enrolled with a registered training organisation and undergoing supervised training towards gaining assessment, and
- under the direct supervision of a person who is licensed to carry out the high risk work.
You can use this licence in all states and territories of Australia.
Add an additional class(es)
You can also add an additional class(es) to an existing high risk licence once you have completed a recognised course of training with a registered training organisation and been formally assessed as competent.
If your high risk work licence is lost, damaged or stolen, you will also need to complete the application form (PDF, 344KB), including Section 4 which captures the reason for the replacement.

What you need to apply, replace, transfer or add a class
When you visit the service centre you must:
- bring a completed Licence to Perform High Risk Application form ( PDF, 344KB)
- bring your current assessment and interim summary assessment from an accredited assessor (the original white copy which needs to be within 60 days of issue date)
- bring your identity documents
- pay the licence fee, and
- have your photo taken.
If you have moved to Tasmania and are applying to transfer your interstate high risk work licence, you must:
- bring a completed Licence to Perform High Risk Application form ( PDF, 344KB)
- bring your current interstate issued high risk work licence (a copy will be taken)
- bring your identity documents
- pay the licence fee , and
- have your photo taken.
If you are adding a class(es) to your high risk work licence, you must:
- bring a completed Licence to Perform High Risk Application form ( PDF, 344KB)
- bring your current assessment and interim summary assessment from an accredited assessor for the new class (the original white copy which needs to be within 60 days of issue date)
- bring your identity documents
- pay the licence fee.
If you are replacing your high risk work licence, you must:
- bring a completed Licence to Perform High Risk Application form ( PDF, 344KB)
- bring your identity documents
- pay the licence fee.
Can I apply, replace transfer or add a class?
You can apply for a high risk work licence if you:
- are at least 18 years of age
- have completed a recognised course of training with a registered training organisation (RTO)
- can use English at a level that enables the safe performance of high risk work
- have the knowledge and ability necessary to safely perform the high risk work
- have been assessed as competent by an accredited assessor.

Proving your identity
You must provide 2 documents from the following lists to support your application. All documents must be:
- current
- the originals (not photocopies or certified copies)
One identity document must contain a photo.
- One document from category A and one document from category B, or
- Two documents from category A.
- Australian Birth certificate (not an extract)
- Australian citizenship papers
- Australian Passport (can be expired up to 2 years)
- Overseas passport/visa
- Australian Driver licence
- Australian Firearm licence
- Tasmanian Government personal identification card (showing full name)
- Department Home Affairs travel documents or Australian Government issued Immicard
- Department of Home Affairs Certificate of Evidence of Resident Status
- Medicare card
- Plastic bank or credit card, with signature
- Tertiary student card
- Department of Veterans' Affairs or Centrelink pensioner concession card
- Births Deaths and Marriages change of name or marriage certificate (to prove name change)
- Security guard or crowd controller licence
- Government issued personal photo ID card.
- High risk work licence or other photo licence issued by Worksafe Tasmania
When changing your name you must provide:
- One document from category A and one document from category B, or
- Two documents from category A
- Proof of change of name that links the current name on your card to your new name (like an original BDM issued marriage or change of name certificate)
